TERMS OF CANCELLATION: (as required by your Open Gym Membership Agreement)
- CANCELLATION FORM: If you are sure that you would like to cancel your Open Gym Membership, just complete and submit the Cancellation Form below.
- WRITTEN CANCELLATION NOTICE: This will serve as your Written Cancellation Notice as required by your membership agreement. Note that if you have a scheduled Auto-Payment within the next 10-day period, that payment will be processed as scheduled. All payments are non-refundable. You may continue to use your active membership for the remainder of the time-frame that has been paid.
- EMAIL CONFIRMATION RECORD: If you do not receive an email confirmation record for this cancellation request, then your cancellation attempt was not completed successfully, and you will need to resubmit another online cancellation request.
